Healthcare Homes Conference

The DoH commissioned us to garner stakeholder feedback on the issues and challenges faced when implementating the Health Care Homes Program. All stakeholders were brought together by the DoH in a national conference and feedback ascertained during workshop activity. The conference outcomes report complemented the formal evaluation, providing more timely feedback on implementation, successes and areas for improvement. The report findings informed both DoH areas of focus and PHN/HCH practice development activities. We were subsequently asked to develop a measurement framework to capture system learning during implementation. In addition, regular feedback was garnered via survey and focus groups from participating PHNs. Again, reports were provide in close to real time to the DoH.
